JOB DESCRIPTION
RN Case Manager
JOB SUMMARY:
A Registered Nurse who assists the Director of Nursing/Clinical Director and the Administrator / Executive Director with the supervision of hospice services to patients in their place of residence in accordance with physician orders and strives to provide the highest quality of care. Skilled nursing and other therapeutic services furnished are under the supervision of the Clinical Supervisor. The Clinical Supervisor is available at all times during operating hours (or appoints a similarly qualified alternate) and participates in all activities relevant to professional services furnished, including the development of qualifications and assignment of personnel.
QUALIFICATIONS:
1. Graduate of an approved school of professional nursing or therapy and currently licensed in the state(s) in which practicing.
2. Three to five (3-5) years of experience in nursing or therapy, preferred.
3. One (1) year experience in hospice nursing or therapy is preferred.
4. Acceptance of philosophy and goals of Agency.
5. Ability to exercise initiative and independent judgment.
6. Ability to work with individuals to enlist cooperation of many people to perform/achieve a common goal.
RESPONSIBILITIES:
1. Understands and adheres to established Agency policies and procedures.
2. Participates in the development of standards which ensure safe and therapeutically effective service to patient/family.
3. Ability to supervise, guide and develop nursing skills and performances.
4. Consults with the Director of Nursing/Clinical Director/Administrator / Executive Director in determining a staffing pattern which will accomplish stated objectives and promote a maximum level of utilization of health personnel.
5. Assists in recruiting, hiring, evaluating and terminating nursing personnel. Assists in instituting, interpreting and documenting disciplinary action.
6. Responsible for the orientation of new patient care personnel.
7. Assists in the planning and consultative needs of staff.
8. Assists in the preparation and implementation of policies and procedures which meet Medicare, Medicaid, The Joint Commission, state and local laws.
9. Displays a willingness to support policies and procedures and uses appropriate channels for changes of such policies.
10. Assists in the coordination of interdisciplinary team services.
11. Assists in the organization of nursing services to delineate and delegate functional responsibility, lines of relationship and communication to provide safe and therapeutically effective service.
12. Participates in the coordination of Agency’s services with the services of other community agencies.
13. Participates in studies and research and other administrative functions as assigned.
14. Serves as a role model for all colleagues by setting an example of high standards in dress, conduct, cooperation and job performances.
15. Observes confidentiality and safeguards all patient related information.
16. Accepts responsibility for regular attendance and punctuality; fulfills job related requirements without regard to time involved.
17. Serves as a resource person to employees.
18. Develops a cooperative relationship and communicates effectively and professionally with physicians.
19. Investigates and reports any problem relating to patient care and/or employee well-being.
20. Immediately reports any accident, incident, lost articles or unusual occurrence to the Director of Nursing/Clinical Director/Administrator / Executive Director.
21. Attends pertinent continuing education programs other than routine in-services and shares information with staff.
22. Monitors equipment for appropriate use and takes steps to keep misuse to a minimum.
23. Assists with the Agency’s ongoing quality assurance performance improvement (QAPI) program.
24. Assists with the development, implementation and evaluation of patient plans of care.
25. Reviews patient medical records for compliance with federal, state, The Joint Commission, local and Agency policies and guidelines.
26. Assists Director of Nursing/Clinical Director in conducting patient care conferences, staff meetings and maintains documentation. Participates in community education projects.
27. Ensures that all necessary supplies and equipment are available.
28. Must be able to perform nursing functions in times of staff shortage.
29. Available to share on-call with nursing personnel as needed.
30. Ensures that documentation is completed in accordance with Agency policy.
31. Assists with staffing visit assignments and on-call.
32. Other duties as assigned by the Director of Nursing/Clinical Director/Administrator / Executive Director.
33. Acts as Director of Nursing/Clinical Director in the Director’s absence.
WORKING ENVIRONMENT:
Works indoors in agency office and is expected to assist at time with the provision of patient care, in the patient homes and travels to/from patient homes.
JOB RELATIONSHIPS:
1. Supervised by: Director of Nursing/Clinical Director
2. Workers Supervised: Nursing staff, hospice aides, medical social workers, chaplain or spiritual care staff, volunteer coordinator.

Ability to perform the following tasks if necessary:
· Ability to participate in physical activity.
· Ability to work for extended period of time while standing and being involved in physical activity.
· Moderate lifting.
· Ability to do extensive bending, lifting and standing on a regular basis.
Job Types: Full-time, Part-time
Pay: $80,000.00 – $90,000.00 per year
Expected hours: 20 – 40 per week
