The RN - Clinical Director will oversee the clinical and financial operations of the multi-disciplinary staff, ensure care is provided to the ultimate satisfaction of patients and their families ensuring clinical appropriateness and quality of patient care according to Medicare regulations.  Works in collaboration with the Medical Director and the Senior Management team regarding the patient care process for the program.  Identifies opportunities in resource management and utilization by oversight and review of clinical operations.

RN / Clinical Educators (Hospice / Home Health) - Austin & Houston, TX
Base Salary: $65,000 to $74,000

Outstanding opportunity with a national leader delivering home based care, working with home health and hospice agencies ensuring patients receive continuity of care in the home environment.  Clinical Educators work directly with patients in their homes, and collaborate with physicians, home health and hospice agencies to establish in-home treatment plans and certify the need for services. Our client partners with home health and hospice agencies, collaborates with independent and assisted living communities, and works with skilled nursing facilities and hospitals nationally to coordinate services and patient transitions to home care.

Candidates must be a licensed Registered Nurse, and have 2+ years Hospice Industry experience, no exceptions.  Home Health experience is a plus.  Base salary is negotiable, based on your experience.  Job details below.
 
QUALIFICATIONS: 
Licensed Registered Nurse with 2+ years hospice experience required
RN home health experience is a plus
Knowledge of Medicare regulations and hospice standards

Required Knowledge/Skills:
Ability to train and develop employees to meet company expectations.
Must be very structured, organized, very detailed and able to meet deadlines.
Working knowledge of computer programs (email, Word, Excel, PowerPoint, etc.
Maintains current knowledge of Medicare Guidelines for Home Care and Hospice and criteria for Home Care and Hospice referral.
Understanding of adult learning principles.
Exhibits excellent interpersonal skills.
Experience with small group presentations, teaching and training. 
Ability to work independently (highly autonomous role)

MAJOR DUTIES AND RESPONSIBILITIES:
Clinical Educator serving continuum of care, works closely with assigned physicians
Provides on-site 1-on-1 clinical coordination / education with patient and families.
Facilitates positive relationship development among the continuum.
Demonstrates maturity in communication and conflict resolution.
Interacts with physicians, staff, patients/families, community agencies, and serves as an educational resource regarding hospice and home care benefits.
Case conferencing to coordinate care, improve documentation and communication
Assists with documentation to support eligibility of patient under the care of hospice or home care.
Weekly and monthly reports completed and turned in to meet time management deadlines
Coordinates paperwork regarding re-certifications and/or prevention of discharge regarding hospice documentation as needed
Reviews patient medical records / Chart Audits
Attends all required meetings (monthly staff, etc.) and in-services.
Attends continuum meetings to facilitate appropriate utilization of continuum resources to meet patient and family needs.

RN CASE MANAGER:

We are in the start-up phase in Las Cruces NM and are currently looking to hire an RN for case management / home visits. The main stipulation is that we need an RN fluent in Spanish, as that encompasses a large part of our patient population. Other than that, it's the basic RN position in visiting patients in their homes, monitoring vitals and assessing pain, working with family and caregivers.
